How We Use Your Information

We use personal information only for purposes a reasonable person would consider appropriate, including to assess care needs, build individualized care plans, schedule and supervise Personal Support Worker (PSW) visits, communicate with families, and meet legal and billing obligations. Consent is obtained from the client or, where applicable, a substitute decision-maker, in line with PHIPA and PIPEDA.

We share information only with authorized caregivers, healthcare providers involved in your care, necessary service providers (processors) bound by confidentiality obligations, or as required by law. We never sell your information.

Substitute Decision-Makers

Where a client cannot make a particular decision, we work with a legally authorized substitute decision-maker (SDM) under Ontario's Health Care Consent Act and Substitute Decisions Act, while continuing to respect and include the client's own voice wherever possible.

If you suspect a privacy breach or have a concern, please contact our Privacy Officer right away. If a concern remains unresolved, you may escalate it to the IPC at www.ipc.on.ca, or to the federal Office of the Privacy Commissioner of Canada at www.priv.gc.ca where PIPEDA applies.
