We collect personal information — and, where relevant to care, personal health information — directly from clients, family members, substitute decision-makers, referral sources, and through our website. This may include:
- Contact and emergency contact details
- Billing and payment information
- Care preferences, routines, and cultural or language needs
- Personal health information needed to safely deliver care
- Communications with our staff and caregivers
- Website form submissions (contact, care assessment, or career applications)
How We Use Your Information
We use personal information only for purposes a reasonable person would consider appropriate, including to assess care needs, build individualized care plans, schedule and supervise Personal Support Worker (PSW) visits, communicate with families, and meet legal and billing obligations. Consent is obtained from the client or, where applicable, a substitute decision-maker, in line with PHIPA and PIPEDA.
How We Protect Your Information
We safeguard your information through:
- Administrative safeguards — staff training, confidentiality agreements, and role-based access
- Physical safeguards — secured storage of records and devices
- Technical safeguards — encryption and access controls
Sharing Your Information
We share information only with authorized caregivers, healthcare providers involved in your care, necessary service providers (processors) bound by confidentiality obligations, or as required by law. We never sell your information.

Our website, ntongocare.ca, may use cookies and analytics tools to understand how visitors use our site and to improve your experience. We do not use cookies to make automated decisions about care eligibility or pricing, and we will seek appropriate consent for non-essential cookies where required.
Substitute Decision-Makers
Where a client cannot make a particular decision, we work with a legally authorized substitute decision-maker (SDM) under Ontario's Health Care Consent Act and Substitute Decisions Act, while continuing to respect and include the client's own voice wherever possible.
Your Privacy Rights
You have the right to:
- Access most personal information we hold about you, subject to limited legal exceptions
- Request correction of inaccurate or incomplete information
- Withdraw or vary consent, subject to legal and care-safety limitations
- Ask questions about how your information is used or shared
We respond to access and correction requests within the timelines required by PHIPA and PIPEDA — generally within 30 days.
Privacy Concerns or Breaches
If you suspect a privacy breach or have a concern, please contact our Privacy Officer right away. We will contain, investigate, and, where required, notify affected individuals and Ontario's Information and Privacy Commissioner (IPC). If a concern remains unresolved, you may escalate it to the IPC at www.ipc.on.ca, or to the federal Office of the Privacy Commissioner of Canada at www.priv.gc.ca where PIPEDA applies.
